To Register

• Complete Youth Registration.
• Families of 3 or more players, use the Family Registration.
• Print Registration Confirmation.
• Sign waiver.
• If necessary, attach Proof of Age.
• Attach required fees.
• Cheques made payable to Cordova Bay Soccer Club.

A volunteer fee is included in your registration. To be reimbursed for volunteering please send an email to Angela Brinkworth at
angelabrinkworth@telus.net indicating when you would be available to volunteer in the concession.

Uniform deposit cheque must be post-dated April 1, 2009

Registration is not complete until a signed waiver and all fees have been submitted to the Club.
